Bess Truman Family Medicine Clinic at TMC Lakewood Renews NCQA Recognition

The National Committee for Quality Assurance is a private, 501(c)(3) not-for-profit organization dedicated to improving health care quality. Since its founding in 1990, NCQA has been a central figure in driving improvement throughout the health care system, helping to elevate the issue of health care quality to the top of the national agenda.

The Bess Truman Family Medicine Clinic at TMC Lakewood has achieved Level 3 (the highest) recognition from the NCQA Patient-Centered Medical Home Program. The program emphasizes evidence-based, patient-centered processes that focus on highly coordinated care and long-term relationships. The program promotes partnerships between individual patients and their personal clinicians. Each patient’s care is delivered by clinician-led teams that provide all health care needs and coordinate treatments across the health care system.

The clinic achieved our first recognition in 2013 and renewed in 2016. The process demonstrates our ability to meet the program’s key elements, embodying characteristics established by the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics, and the American Osteopathic Association. These components include:

  • Written standards for patient access and continuity of care.
  • Use of patient feedback materials.
  • Appropriate use of charting tools to track patients and organize clinical information.
  • Responsive care management techniques with an emphasis on preventive care
  • Adaptation to patients’ cultural and language needs.
  • Use of information technology for prescriptions, tests, care coordination and referral tracking.
  • Use of evidence-based guidelines to treat chronic conditions.
  • Measurement and reporting of clinical and service performance.

This recognition was achieved through our ongoing commitment to the principles of the NCQA PCMH program and through the hard work of our dedicated clinicians and staff as well as the valuable input from our patients.
